I got the old pals quest line by beating zoltan chivay in gwent. First we had to start the old pals quest by playing zoltan, then move to roche, and only after that could we challenge thaler. I caught him walking around novigrad. The rewarded in this quest is unaffected by the ±5 level constraint and difficulty level. Or can i play him in gwent anytime after i start old pals (assuming he's still in novigrad)?

If you finish the quests without winning his card, you can find him at the seven cats inn outside novigrad. Some npcs are no longer available or dead.

There does seem to be one missable gwent card that you get at the end of the gwent card game tutorial in the bar at the start of the game.
